Overview
Come work with a great team! Signet Health manages the behavioral health services for Medstar Washington Hospital Center (MWHC) and has a Full-time Day Shift Ambulatory Behavioral Health Care Coordinator.
This position will support the Outpatient Behavioral Health Clinical Team, assist with difficult patients of high acuity to ensure patient safety, compliance with treatment, and care coordination between care teams.The position will report to the Outpatient Behavioral Health Clinical Director.
Job Responsibilities
Assists in coordinating patient referrals to the outpatient behavioral health service.
Coordinates the transition between different levels of psychiatric care throughout programs at center, including partial hospitalization, intensive outpatient, group therapy, individual therapy, and medication management.
Supports discharge planning for all outpatient behavioral health levels of care, including providing resources and linking patients to community services.
Manages a small short-term caseload of patients with severe and persistent mental illness or who have high acuity; this could be a panel of some of the same patients seen by the residents. Caseload is monitored and approved by the Clinical Director. Case management tasks include:
Making safety-check telephone contacts with patient
Assist patients in keeping behavioral health appointments and requesting medication refills
Position would assist residents in managing patients on their caseload.
Position would assist in scheduling appropriate patients with the residents
Provides resources for basic needs, like food banks, transportation, etc.
Assists with linking to Core Service Agencies
Serving as a liaison between other care teams to ensure continuity of care
Participates in interdisciplinary treatment planning, as needed.
Works with a variety of internal and external customers to ensure the delivery of high-quality care with excellence in clinical outcomes and customer satisfaction.
Assists in coordinating insurance benefits and getting Emergency Medicaid, if needed
Competencies
Proficiency in interpersonal communication and interdisciplinary teamwork.
Proficiency with creating and maintaining spreadsheets and electronic computer files.
Knowledge of community resources in the District of Columbia, Maryland, and Virginia
Skill in providing care to patients with various types and presentations of psychiatric/behavioral health disorders.
Requirements/Qualifications
Candidates should have a minimum of 2 years’ experience working with psychiatric/behavioral health patients. Must be flexible to adjust to the needs of a fast-paced environment, demonstrate effective problem-solving skills and display excellent oral/written communication skills. Adult behavioral health and substance use treatment experience is preferred. Master’s degree in social work and/or Bachelor’s degree preferred.
